- Group name and size (# of employees)
- Company location (City, State, Zip)
- Desired plan options (PPO, HMO, dual-option, etc.),as well as approximate deductible, coinsurance, Rx, and/or premium levels (In order to help us narrow the field of hundreds of products).
- Lastly, we need a complete census with information on all eligible employees applying for coverage.
